My Emergency Contact Card

Knowing Who Can Help

Knowing who can help you during an emergency can make it easier to get help quickly.

Complete this worksheet with a parent, teacher, or trusted adult.

Remember:

KNOW → CALL → TELL

KNOW — Know which trusted adults can help.

CALL — Know that an adult can help contact emergency services when needed.

TELL — Be ready to tell a responsible adult what happened and where help is needed.

3. WHAT INFORMATION SHOULD I TELL?

When you ask for help, try to explain three important things.

WHAT happened?

WHERE am I?

For practice, you can use a made-up place instead of your real home address.

WHO needs help?

4. PRACTISE SAYING IT

Use a made-up practice situation.

Complete the sentence:

“I need help. happened. I am at .”

Practise saying your sentence with a trusted adult.

5. MY LOCAL EMERGENCY NUMBER

Ask a parent, guardian, or trusted adult which emergency number to use where you live.

My local emergency number:

Follow your family’s or school’s emergency procedures.

6. IF I AM SEPARATED FROM MY FAMILY

If you become separated from your family:

Stay calm.

Stay in a safe place.

Look for a responsible adult who can help.

Do not leave with an unknown person simply because they offer help.

If possible, seek help from appropriate staff, security, or another responsible adult in the location.

You do not have to solve the emergency by yourself. Ask a responsible adult for help.

8. QUICK SAFETY REMINDER

FOR ADULTS

For adults: Complete the contact information together with the child. Check that the numbers are current and explain when and how the child should seek adult help.
